These will be included in edits to the main entries, but here are the big news:
1. Men's Boots (Crew Uniform):
These appear to have been off-the-rack black DKNY "Rebels" with a piece of leather to cover the zipper. Not only has at least one extra mentioned this, but there are good pictures of these boots (see the end of this entry) and they match the "Rebel" boot nicely.
"DKNY Rebels" in Brown:

They're discontinued and rare, but sometimes brown ones have appeared for sale. One black pair was on ebay recently with a starting bid of $300 (reserve not met) and it didn't get any bids (including none from me! Too rich for my blood.) For reference, these boots originally sold for $162 Cdn on Overstock.com.

2. Women's Boots (Crew Uniform):
Sadly, these do not have such great pictures of them. To add to the confusion, the boots pictured seem to vary. Steve at the 1701st posted a nice comparison of some screencaps and boots on display:

Probably one of the best pictures of Uhura's boots is from the cardboard standee:

MirkwoodsQueen on the 1701st saw a pair on display in Philadelphia and described them as "smooth black leather, with a front seam, a horizontal seam at the ankle, and a toe cap with topstitching. The boot vamp also had extra pieces seamed under/beside the toe cap (like a "V" that starts at the toe)."
(Edit added June 23): I've found a couple screencaps of the scene with Uhura removing her boots, and the one design detail that stands out for me on these is a section of vertical grooves/bars at the top of the boot. However, she's dressed as a Cadet in this scene, so these could be entirely different boots from the Crew boots.

The women's DKNY boot that corresponded to the "Rebel" looks like it was the "Dia" and you might still be able to find a pair out there. The DKNY "Hyper" boot is also pretty similar. These look great though they don't appear to have the vertical bars at the top:

I have to admit I'm not looking super-close at these boots since I don't need a pair, but I've noticed some nice "Romantic Soles" boots that I think have a good look to them:

3. Crew Pants (both men and women):
Eagle eyes on a couple forums have independently noticed the Hein Gericke brand motorcyle pants called the "Speedy". These are so close to the screen-worn pants I strongly suspect they were either used (perhaps just for the extras) or served as design inspiration. Recall that the costume designer described them as being dyed charcoal grey (which implies they either made or at least modified some of them) and as a sort of heavy work jeans.

Before you get too excited, these were discontinued long ago, and Hein Gericke no longer even imports to North America. The German (and UK) catalogs no longer have this style, nor anything like them. I've contacted every motorcycle apparel place in Calgary, and a few in Vancouver and Quebec without any luck.
You can still find the occasional women's pair out there though, so if you're looking to recreate the women's pant suit, then snap them up now!
The "Speedy"s are textile pants, and while motorcycle textile pants are often denim these ones were made of "waxed oxford cotton" and looked like a canvas or "duck" cotton. I imagine that sort of heavy material that work pants and overalls are made out of. The knee was described as "power stretch".
The suspicion is that the Captain Pike Costume Card has samples of the pant material, and I'll be posting my study of the Pike card soon.

You can see some differences between the Speedy and the pants Kirk and Spock wear. The back pockets in the movie are welt pockets, the Speedy has zippered patch pockets. The Speedy's have a little belt thing below the waistband on the back, the movie pants don't. The ribbed section looks a little bit different. The Speedy pants have rivets, the movie pants don't appear to. There looks like there's an extra seam below the knee on the movie pants.
